Applying for services

STEP 3: The admissions office will contact you to schedule an intake interview and a tour of your program of interest.

STEP 4: Once the application is received and the intake interview is complete, our team will determine eligibility, capacity, and the services we can offer you.

STEP 5: GET STARTED! We will be in touch and coordinate service access.

Brief Service Descriptions and Criteria (click to expand)

Intensive Residential Treatment Services (IRTS)

Rooted in the Fairweather Lodge model, the Tasks Unlimited Training Center (TUTC), our licensed Intensive Residential Treatment Services (IRTS), provides adults with symptom stabilization, group and independent living skills practice, resource coordination, mental and chemical health education, medication support, and recreational opportunities. This nine-bed, co-ed program emphasizes peer support and employment as keys to mental wellness and stability. Many people move into our Transition Lodge when they leave TUTC.

Eligibility criteria:

Jobs Training Program (JTP)

A 12-week program that provides training in skills needed for employment that focuses on mental health management, peer support, and independent living skills. This program offers training curriculum with a weekly job group that discusses topics including mental health at work, job searching, finances, and more. The program has two tracks that can provide either paid on the job janitorial training, or support finding a job in the community. 

Eligibility criteria:

  • 18 or older
  • Diagnosis of Severe and Persistent Mental Illness (SPMI) or willing to complete a diagnostic assessment
  • Current Hennepin or Dakota Resident (or approved out of county agreement)
Job Retention Services (Extended Employment)

The purpose of Extended Employment is to provide the ongoing support services necessary for Minnesotans with disabilities and other significant barriers to employment to maintain and advance in jobs and careers.  Note that this program is not available for waivered persons, as most waivers (including CADI) provide employment support as part of the waiver.  Also, note that this program is appropriate for adults who already have employment, but may need assistance maintaining employment.

Eligibility criteria:

  • 18 or older  
  • Disability diagnosis 
  • Cannot be receiving waivered services (since they already include employment support options)
Lodges

The Fairweather Lodge model offers a unique, community-based approach to mental health recovery and independent living. Lodges are shared homes for 3–8 “members”, fostering an extended-family environment where individuals support one another and share responsibility for managing their household. This peer-driven model promotes teamwork, self-reliance, and personal independence.

Eligibility criteria:

  • 18 or older
  • Diagnosis of Severe and Persistent Mental Illness (SPMI) or willing to complete a diagnostic assessment with one of our clinicians or external provider 
  • Hennepin or Dakota Resident (an out of county agreement may be available in some situations)  
Northeast Outreach and Opportunity Center (NOOC)

Emergency Services Program (ESP) Drop-in center for anyone who is experiencing homelessness or housing instability. Do not need to be a resident of a particular county and there is no diagnosis requirement to be enrolled into the NOOC. Services: food, clothing, transportation assistance and housing resources and shelter resources for clients. 

Eligibility criteria:

  • 18 or older
  • Currently homeless or unstably housed
Program for Assistance in Transition from Homelessness (PATH)

Services for people with mental illness who are currently experiencing homelessness. The PATH team works to connect eligible individuals with mental health treatment, substance use services, housing assistance, and other supports to help them transition out of homelessness.

Eligibility criteria:

  • 18 or older  
  • Diagnosis of mental illness  
  • Hennepin, Dakota, or Ramsey County resident
  • Currently homeless or unstably housed
The Resource Center (Community Support Program)

The Resource Center offers 1-on-1 coordination and peer support services to ensure people are connected with the resources they need for successful mental health recovery.

Eligibility criteria:

  • 18 or older
  • Resident of Hennepin County
  • Diagnosis of mental illness
Targeted Case Management

Targeted Case Management (TCM) offers support in identifying needs, setting goals, and creating a plan for stability and wellness. It provides help connecting clients to resources such as therapy, housing, medical care, benefits, and community supports. TCM also offers ongoing advocacy and monitoring to reduce barriers and keep clients progressing toward their goals.

Eligibility criteria:

  • 18 or older  
  • Diagnosis of mental illness  
  • Hennepin, Dakota, or Ramsey County resident
